| Applications accepted | Child's medical statement - this is a mandated form by the state of ohio. a child must have a physical with a physician on a yearly basis to stay enrolled at the ccd. child enrollment and health information - this form is also mandated by the state of ohio. this form gives the center health information and transportation permission in the event of an emergency. it should also state any food restrictions or allergies. a copy of this form accompanies children on all field trips. please make sure both sides are completed |
