| Applications accepted | 3's children who are toilet-trained and have, or will have, reached their 3rd birthday by august 1st of the year they enter the program. 4's children who are toilet-trained and have or will have reached their 4th birthday by december 31st of the year they enter the program. pre-k children who are toilet-trained and have, or will have, reached their 5th birthday between august 1 and december 31 of the year they enter the program. state of hawaii health form 14: available and completed by a licensed physician as proof of physical examination within the last year, update of immunizations, description of any allergy, and a t.b. clearance. this form is required prior to the 1st day of school. a 3's physical exam and immunization record is still valid when entering the 4's program. all new entering 3's and 4's need a form 14. we have an open enrollment policy of accepting children into our program at any time of the year if the child meets the requirements, and if there is space available. |
