| Applications accepted | To start the enrollment process, simply contact the head start location nearest you and schedule an appointment. proof of income (pay stub and/or proof of public assistance) certified birth record for each child current immunization records current medical examination report (within one year) social security number for each family member to enroll your child in a head start program, your child must be four years old or at least three years old by december 1. |
