| Applications accepted | Admission is subject to completion of an application for admission, an interview with the director, and payment of the registration fee and the first tuition payment. a current immunization record and physical examination form must be on file within one week after entrance to school. the pick-up authorization, child?s history form, enrollment supplement form, field trip participation form, alternate nutrition agreement [if applicable], record of any allergies, and topical medication form must be on file by the date of entrance. |
