| Applications accepted | interview with our director. the following documents must accompany your application: copy of child's birth certificate, copy of child's social security card, immunization certificate (ga form 3231), and for children who are age 4 or over, or applying for ga pre-k program: an eye, ear and dental examination certificate (ga form 3300). form 3231 must have either the "date of expiration" or "school attendance" block checked. children who attend pre-k must be up-to-date on all immunizations required for school entry plus haemophilus influenza type b (hib) vaccine. |
